Description
Posteriorly, the scalp and posterior neck vasculature frames the occiput and upper cervical spine, with paired vertebral arteries ascending through the transverse foramina to enter the cranial cavity and contribute to the basilar artery along the ventral brainstem. Superficial venous channels over the occipital region and posterior auricular territory drain inferiorly toward the suboccipital and deep cervical pathways, while deeper veins track alongside the vertebral column to the brachiocephalic veins at the thoracic inlet. Laterally, the internal and external carotid systems are suggested by their parallel course in the neck, but the emphasis remains on the dorsal conduits and their symmetry around the midline. A posterior perspective matters because it clarifies what is otherwise hidden in standard anterior neck diagrams: the vertebrobasilar circulation and the venous routes that communicate with the dural venous sinuses and cervical plexuses. This is the road map for vertebral artery dissection and posterior circulation ischemia, and it also explains why high cervical manipulation, atlantoaxial trauma, or rotational compression can produce vertigo, diplopia, or brainstem symptoms. Small vessels are not decoration here, they outline collateral channels that become relevant when carotid flow is compromised. Neuroanatomy, gross anatomy, and radiology teaching benefit from this layout when correlating CTA, MRA, or catheter angiography with surface and deep landmarks, including the relationship of vertebral vessels to the cervical vertebrae and the posterior fossa circulation around the brain. It also supports operative and interventional discussions for posterior fossa approaches, cervical spine exposure, and central venous access planning at the lower neck and upper chest.
