Description
Running superiorly from the thorax into the neck, the common carotid arteries bifurcate at the level of the upper thyroid cartilage into internal and external carotid arteries, while the internal jugular veins descend lateral to the carotid sheath and converge inferiorly toward the brachiocephalic veins. Within the cranial vault, the internal carotid circulation is suggested as it contributes to the anterior cerebral and middle cerebral arterial territories, with paired dural venous sinuses draining toward the sigmoid sinus and into the internal jugular vein. Anteriorly, the aortic arch gives rise to the brachiocephalic trunk on the right and the left common carotid and left subclavian arteries, setting the proximal relationships between great vessels, neck vasculature, and the visible heart. Color coding differentiates arteries (red) from veins (blue). Carotid bifurcation anatomy is a daily clinical landmark, and this view makes it easier to teach where atherosclerotic plaque concentrates, why a carotid bruit localizes there, and how endarterectomy or carotid stenting relates to the internal carotid origin and nearby cranial nerves in the carotid sheath. Intracranially, tying arterial supply to venous outflow helps connect stroke patterns in the middle cerebral artery territory with pathways of venous drainage that can be involved in dural sinus thrombosis. The continuity from cranial vessels to the great vessels also supports catheter navigation concepts from the aortic arch into the common carotid and internal carotid arteries.
