Description
Running along the anterior compartment of the arm, the brachial artery continues distally from the axillary artery at the inferior border of teres major and descends medial to the humeral shaft toward the cubital fossa. Proximally it sits anteromedial to the humerus, then approaches the elbow between the brachialis (deep) and biceps brachii (superficial), accompanied by paired brachial veins and with the median nerve crossing from lateral to medial in the mid arm. At the level of the neck of the radius in the cubital fossa, the vessel bifurcates into the radial and ulnar arteries, which course along the anterolateral and anteromedial forearm respectively toward the wrist and hand. Bony landmarks such as the scapula, humerus, radius, ulna, and the carpal and metacarpal framework provide orientation for the surface projection of these vessels. For clinicians, the anterior course of the brachial artery is the practical map for blood pressure measurement and vascular access, since the artery is palpated and auscultated just medial to the biceps tendon in the cubital fossa. It is also the vessel at risk in supracondylar fractures of the humerus, where tethering or spasm can compromise distal perfusion and contribute to acute ischemia and, later, Volkmann contracture. Small but memorable anatomy. Understanding its relationship to the median nerve matters during percutaneous procedures and exploration for traumatic laceration. Educators will find this plate well suited for gross anatomy teaching of the arm (brachium) and forearm, linking osteology to the arterial and superficial venous patterns such as the cephalic vein used for venipuncture and cannulation.
