Description
Arising inferior to the diencephalon, the brainstem is rendered from a posterior (dorsal) perspective, with the mesencephalon (midbrain) superior, the pons central, and the medulla oblongata tapering inferiorly toward the cervical spinal cord. Along the dorsal pons and open medulla, the floor of the fourth ventricle (rhomboid fossa) lies immediately anterior to the cerebellum, framed laterally by the cerebellar peduncles. Superiorly, the region of the superior and inferior colliculi sits dorsal to the cerebral aqueduct, while the cerebellar hemispheres and vermis occupy a posterior position relative to the brainstem in the same field. Spatial context is reinforced by the semi-transparent overlay of the cerebrum and cerebellum around the hindbrain. A posterior brainstem view matters because it is the teaching view for the fourth ventricle and its landmark anatomy, which guides both neuroanatomy localization and operative planning around the dorsal pons and medulla. Lesions compressing the fourth ventricle, such as ependymoma or medulloblastoma in the posterior fossa, distort the rhomboid fossa and cerebellar peduncle relationships in predictable ways. This is also the corridor for the telovelar approach to the fourth ventricle, where respecting the inferior medullary velum and cerebellomedullary fissure helps limit traction on brainstem nuclei.
