Description
Centered in the female upper torso, the trachea descends in the midline from the laryngeal inlet to the carina, where it bifurcates into the right and left main bronchi. The right main bronchus courses more vertically and divides into lobar (secondary) bronchi for the superior, middle, and inferior lobes, while the left main bronchus passes inferolaterally beneath the aortic arch toward the superior and inferior lobar bronchi. Segmental (tertiary) bronchi branch distally within each lung, tapering into smaller conducting airways that fan laterally and inferiorly toward the peripheral parenchyma. Bronchopulmonary segments are implied by this branching geometry. That carinal split matters clinically. The more in-line orientation of the right main bronchus explains why aspirated material and misplaced endotracheal tubes tend to enter the right bronchial tree, often producing right lower lobe atelectasis or post-obstructive pneumonia. This is also the anatomic basis for lobar and segmental localization in bronchoscopy, bronchial blocker placement, and interpretation of mucus plugging patterns in asthma and COPD.
