Description
Posteriorly, the third cervical vertebra (C3) is centered on the vertebral arch, with paired laminae converging in the midline to form the base of the spinous process. Laterally, the laminae continue into the pedicles and the articular pillars, where the superior and inferior articular processes frame the zygapophysial (facet) joints with C2 and C4. The transverse processes project to either side, and the transverse foramina sit anterolateral to the articular pillars, separated from the vertebral foramen by the pedicle. Compact bone contours and the broad cervical vertebral foramen are implied even from a posterior perspective. A posterior C3 view is a workhorse for teaching posterior element anatomy and for orienting the clinician to the junctional region where cervical mobility meets vulnerability. Fractures and stress injuries at the lamina and pars interarticularis, while less common than in the lumbar spine, matter in high energy trauma and in postoperative assessment after posterior decompression or instrumentation. Needle and hardware trajectories in posterior cervical approaches must respect the lateral mass, the facet joints, and the expected course of the vertebral artery through the transverse foramina.
