Description
Posteriorly, the cervical vertebra is presented with the vertebral arch dominating the view, where paired laminae converge in the midline and continue into the spinous process. Lateral to the spinous base, the superior articular processes rise to form the zygapophysial (facet) joints, while the inferior articular processes sit slightly caudal and anterior, partly obscured from a strict posterior perspective. Transverse processes project laterally on both sides, framing the posterior aspect of the vertebral foramen and marking the level where the transverse foramina would sit anterolaterally. For C5 specifically, the bifid spinous tip and stout lamina are common teaching landmarks when differentiating typical mid cervical vertebrae from C1, C2, and C7, and a posterior view makes those distinctions immediate. This is the vertebral level often referenced in cervical spondylosis and facet-mediated neck pain, where degenerative change at the C4 to C5 and C5 to C6 zygapophysial joints can correlate with axial pain and restricted extension. Spine surgeons and interventionalists also work off these posterior bony contours when planning posterior approaches, placing lateral mass screws, or targeting medial branch blocks, where the relationship between the articular pillars and the lamina matters more than the vertebral body.
