Description
Arcing anterolaterally from the coracoid process to the anterior margin of the acromion, the coracoacromial ligament forms a triangular band that roofs the superior aspect of the glenohumeral joint. Deep to it sit the humeral head and greater tubercle, aligned with the glenoid cavity of the scapula and bordered superiorly by the acromion. The proximal humerus is shown in relation to the lateral scapular spine and the anterior thoracic cage, with ribs lying medial and inferior to the shoulder girdle. Bone landmarks read clearly. Clinically, this ligament defines the coracoacromial arch, the structure implicated in subacromial impingement when rotator cuff tendons (most often supraspinatus) and the subacromial-subdeltoid bursa are compressed between the greater tubercle and the undersurface of the arch during abduction and forward flexion. Surgeons reference this anatomy during arthroscopic subacromial decompression and acromioplasty, where release of the coracoacromial ligament can relieve impingement but also risks anterosuperior escape of the humeral head in massive cuff tears. The anterior perspective is useful for orienting the coracoid as a palpable landmark and for understanding why anterior acromial morphology influences symptoms.
