Description

Seen posteriorly, the deep fascia invests the distal leg and continues over the hindfoot as a dense connective tissue sheath blending with the calcaneal (Achilles) tendon and the periosteum of the calcaneus. Superficial to it sit the gastrocnemius bellies and soleus, which converge inferiorly into the tendo calcaneus; the fascial planes define their margins and guide the eye to the midline tendon as it inserts on the posterior calcaneal tuberosity. Inferior and slightly lateral to the tendon, the fascial thickening over the heel frames the calcaneal region while the malleolar contours mark the transition into the ankle. Posterior pedal and ankle fascia matters because it is where force transmission, friction control, and compartment boundaries meet. The paratenon and adjacent deep fascia around the Achilles tendon are common pain generators in midportion tendinopathy and paratendinopathy, and they become key landmarks during ultrasound-guided procedures and open debridement. At the heel, fascial continuity helps explain why Achilles insertional tendinopathy can coexist with retrocalcaneal bursitis and posterior calcaneal prominence (Haglund-type morphology) in runners and jumping athletes.
