Posterior shoulder anatomy is centered on the deltoid fascia as a dense connective tissue sheath investing the deltoid muscle and spanning from the scapular spine and acromion laterally toward the proximal humerus. Superficial to the underlying muscle fibers, the fascia follows the contour of the posterior deltoid as it curves inferolaterally toward the deltoid tuberosity, with the posterior border of the deltoid forming a clear margin against adjacent posterior scapular musculature.
