Description
Cutaneous hair follicle anatomy is rendered as a 3D sectional model with the hair shaft emerging superficially and continuing inferiorly through the follicular canal toward the bulb. Concentric epithelial layers form the follicle wall, with the inner root sheath lying directly adjacent to the cuticle and cortex of the hair and the outer root sheath positioned more peripherally, continuous with the epidermis. At the most inferior pole, the dermal papilla sits centrally within the hair bulb, while small-caliber arteries and veins course in the surrounding dermis to supply the follicular unit. An obliquely oriented arrector pili muscle inserts onto the follicle’s connective tissue sheath and runs superiorly toward the superficial dermis. This view matters for teaching the pilosebaceous unit as an integrated mini-organ rather than an isolated hair. The relationship of the dermal papilla to the hair matrix explains why injury, scarring alopecia, or inflammatory destruction at the bulb can permanently reduce hair regeneration, while more superficial pathology can spare the follicular stem cell niche in the bulge region. It also supports procedural planning: understanding the depth and angulation of the follicle helps reduce transection during follicular unit extraction and guides targeting in intralesional corticosteroid injection for alopecia areata. Use this illustration in dermatology and integumentary-system coursework, in patient-facing education on hair growth cycles, and in surgical or cosmetic medicine materials covering hair transplantation, folliculitis, and pilonidal or ingrown-hair disease mechanisms.
