Description

Across the dorsum of the male hand, the dorsal metacarpal veins course superficially over the metacarpals, running in the intermetacarpal spaces between the extensor tendons and converging proximally toward the dorsal venous network near the wrist. Distally, tributary venules from the dorsal digital veins drain from the fingers toward these metacarpal channels, with vessels crossing obliquely between adjacent rays. Deep to the veins, the metacarpals articulate with the proximal phalanges at the metacarpophalangeal joints and proximally with the carpal bones, providing a bony roadmap for the vascular paths. Arterial pathways are indicated in red, typically tracking deeper and more protected than the dorsal superficial veins. For cannulation and venipuncture, the dorsal metacarpal veins are often the first target because they lie in the superficial fascia and remain accessible even when palmar veins collapse. Placement is not trivial. Overzealous probing near the anatomical snuffbox risks injury to the superficial branch of the radial nerve and, more laterally, the cephalic vein outflow, and extravasation on the dorsum can spread rapidly along loose areolar tissue and produce dramatic swelling. The posterior view also clarifies why dorsal hand edema and superficial thrombophlebitis can obscure landmarks, complicating peripheral IV access.
