Description
Bisecting the head and vertebral canal in the midsagittal plane, the dura mater (pachymeninx) lines the inner table of the calvaria and continues inferiorly through the foramen magnum as the spinal dura, forming a tubular dural sac around the spinal cord. Superiorly and posteriorly, the dural reflections would be appreciable as they separate major venous channels, with falx cerebri interposed between the cerebral hemispheres and tentorium cerebelli spanning above the cerebellum and dorsal brainstem. Deep to the dura, the arachnoid mater and subarachnoid space contour the gyri and sulci of the cerebral cortex and envelop the cerebellum, while the brainstem transitions into the cervical spinal cord anterior to the posterior elements of the vertebrae. Sagittal section is the clearest way to teach how cranial dura differs from spinal dura, including the periosteal and meningeal layers cranially versus the single meningeal layer in the vertebral canal. Those relationships are not academic; they explain why epidural hematomas accumulate between skull and periosteal dura after middle meningeal artery injury, while spinal epidural anesthesia targets the space outside the dural sac, classically via the lumbar interlaminar approach below the conus medullaris. Dural reflections are also the scaffolding for dural venous sinuses, a key concept when correlating sagittal anatomy with cerebral venous thrombosis patterns. Neuroscience and gross anatomy courses use this artwork to orient students to meninges, dural partitions, and the craniovertebral junction before moving to neuroimaging planes and clinical cases.
