Description
Endometrium occupies the inner mucosal layer of the uterus, arranged as a gland-rich lining overlying the myometrium. Within the frame, tubular endometrial glands course perpendicular to the luminal surface, embedded in a vascularized cellular stroma that contains a branching microvascular bed, including the spiral arterioles supplying the superficial functionalis. Deeper tissue suggests the basalis at the endometrial–myometrial junction, where glands and stroma appear denser and more regenerative, while the more superficial compartment reads as the zone that thickens and sheds across the menstrual cycle. Cyclic remodeling of this tissue under estrogen and progesterone drives the classic teaching distinctions between proliferative and secretory patterns, and the gland-to-stroma relationship is where those patterns become obvious. Irregularly crowded glands with reduced intervening stroma raise concern for endometrial hyperplasia and possible progression to endometrioid adenocarcinoma, while an atrophic endometrium in postmenopausal bleeding shifts attention to focal lesions such as polyps or carcinoma. Light and contrast emphasize gland lumina, stromal cellularity, and the vascular component, supporting side-by-side comparison with endometrial biopsy findings and standard histology texts.
