Description
Running along the dorsolateral forearm, the extensor digitorum originates from the lateral epicondyle via the common extensor tendon and forms a broad muscle belly that lies posterior to the brachioradialis and extensor carpi radialis longus on the radial side. Distally, its tendon bundle narrows and passes deep to the extensor retinaculum, then separates into slips that course over the dorsal aspect of the wrist and hand toward digits two through five. From a lateral perspective, the brachioradialis frames the anterior border, while extensor carpi ulnaris and the posterior ulna sit more posterior and medial, clarifying how the extensor digitorum occupies the central portion of the superficial extensor compartment. Lateral views matter when teaching the crowded topography around the lateral epicondyle and the proximal forearm, where extensor digitorum shares the common extensor origin with extensor digiti minimi and extensor carpi ulnaris and lies near the course of the posterior interosseous nerve as it enters the supinator. This is the anatomy behind refractory lateral epicondylalgia and radial tunnel syndrome, and it also explains why surgical exposures and injections placed too anterior risk drifting toward the brachioradialis and radial artery rather than the extensor mass. Extensor tendon lacerations over the dorsum of the hand become easier to conceptualize when you can trace each slip proximally under the retinaculum.
