Description

Centered on the posterior (dorsal) forearm of an adult male, the extensor digitorum occupies much of the central superficial compartment, arising from the lateral epicondyle and narrowing distally into tendinous slips that course toward digits II to V. Lateral to it, the extensor carpi radialis brevis is emphasized in red, running deep to extensor carpi radialis longus as it heads from the common extensor origin toward the base of the third metacarpal. Medially, the extensor digiti minimi and extensor carpi ulnaris typically bracket the extensor digitorum, with their tendons converging toward the dorsum of the wrist beneath the extensor retinaculum. Four tendons. Clear separation begins proximal to the wrist. Posterior forearm anatomy is where tendon topography meets common injury patterns, and this view makes the relationships around the lateral epicondyle easy to teach and to recognize. Extensor carpi radialis brevis is the tendon most consistently implicated in lateral epicondylitis (tennis elbow), and its proximity to the extensor digitorum at the common extensor origin explains why pain and weakness often involve both wrist extension and finger extension during resisted gripping. Distally, the rapid transition of extensor digitorum into multiple tendons foreshadows zones where dorsal wrist lacerations, extensor retinaculum constriction, or tenosynovitis can disrupt independent finger extension.
