Description
Arising from the anterior division of the internal iliac artery, the uterine artery courses medially within the base of the broad ligament toward the cervix, then ascends along the lateral uterine wall toward the fundus. Along this ascending segment, arcuate arteries run circumferentially within the myometrium and give off radial arteries that pass perpendicularly toward the endometrium, accompanied by closely paired venous channels that coalesce into the uterine venous plexus. Superiorly, anastomoses with the ovarian artery are implied near the cornual region, while inferiorly the cervical and vaginal branches communicate across the cervicovaginal junction. Veins track the arteries but form a more plexiform network. This arrangement matters in the operating room. During hysterectomy and cesarean delivery, the uterine artery is ligated near the cervix where it crosses superior to the ureter (the classic water under the bridge relationship), and uncontrolled bleeding often reflects failure to control the ascending uterine segment or the venous plexus within the broad ligament. Radial and spiral artery remodeling also sits at the center of postpartum hemorrhage and abnormal uterine bleeding, where impaired uterine tone or placental bed pathology prevents effective closure of these channels.
