Description
Anterior forearm fascia is centered on the lateral (radial) side, forming the investing sleeve over the mobile wad of Henry. Beneath this superficial layer, brachioradialis lies most lateral and superficial, with extensor carpi radialis longus and extensor carpi radialis brevis positioned just posterior and slightly medial as they course distally toward the radial styloid and dorsal bases of the second and third metacarpals. The fascia blends proximally with brachial fascia and the lateral intermuscular septum, and it tightens distally as it approaches the extensor retinaculum at the wrist, keeping these extensor-supinator structures closely applied to the radius. Mobile wad fascia matters because it frames a frequent pain generator and a surgical corridor. The interval between brachioradialis and extensor carpi radialis longus/brevis is a practical landmark in the dorsal (Kocher) approach to the proximal radius, and awareness of the fascial planes helps protect the posterior interosseous nerve as it passes through the supinator (Arcade of Frohse) when the forearm is pronated. Lateral epicondylitis often involves extensor carpi radialis brevis near the lateral epicondyle, and fascial thickening or scarring can complicate dissection and obscure tendon origins.
