Description
Anteriorly in the midline pelvis, the adult female urinary bladder is opened in sagittal section so the mucosa lines the lumen from the bladder dome (superior) down to the bladder neck (inferior). The mucosal layer (urothelium with underlying lamina propria) is shown draping over the detrusor muscle, with mucosal folds that flatten toward the trigone, the smooth triangular region at the posteroinferior base. Inferiorly, the bladder neck continues into the proximal urethra, while the posterior bladder wall lies adjacent to the anterior vaginal wall and cervix region in typical female pelvic relationships. Sagittal bladder mucosa matters because much of cystoscopic decision-making is mucosa-first. Gross hematuria workups, suspected carcinoma in situ, and assessment of inflammatory change (interstitial cystitis and recurrent cystitis) all hinge on recognizing where mucosal thickening, erythema, ulceration, or a papillary lesion sits relative to the trigone and bladder neck. Short. The trigone’s smoother mucosa and fixed position contrast with the rugae of the bladder body, a distinction that helps explain why tumors, diverticula, and iatrogenic injury risk often cluster around the base and ureteric orifices during instrumentation.
