Description
Running along the lateral compartment of the male leg, the fibularis (peroneus) longus forms a superficial muscle belly that arises from the head and proximal lateral surface of the fibula and the adjacent intermuscular septa. Distally, its tendon courses inferiorly and posteriorly to the lateral malleolus of the fibula, lying posterior to the fibularis brevis tendon under the superior and inferior fibular (peroneal) retinacula. At the level of the calcaneus and cuboid, the tendon turns plantarward through the groove on the cuboid and then passes medially across the sole toward its insertion on the base of the first metatarsal and medial cuneiform. The tibia sits medial to this muscle-tendon unit, framing the lateral border of the crural and talocrural anatomy. Clean lateral landmarks. This lateral perspective matters when teaching ankle eversion mechanics and the lateral stabilizers of the hindfoot, because the fibularis longus functions as an evertor and plantarflexor and also supports the transverse arch as it crosses the plantar midfoot. The peroneal tendons commonly subluxate after injury to the superior fibular retinaculum, and tears of the fibularis longus tendon often occur near the cuboid tunnel where friction and compression increase with repetitive loading. Surgeons also orient to the fibular groove behind the lateral malleolus during peroneal tendon exploration and retinacular repair.
