Description
Running along the lateral compartment of the leg, the fibularis (peroneus) brevis forms a thick muscle belly on the distal fibula, deep and slightly anterior to fibularis longus as it approaches the ankle. Its tendon narrows distally, passes posterior to the lateral malleolus in the retromalleolar groove, and continues inferiorly and anteriorly toward the lateral aspect of the foot. Adjacent bony landmarks include the distal fibula, talus, and calcaneus, with the tibia positioned medial to the fibula at the ankle mortise. Joint spaces at the tibiotalar and subtalar articulations are outlined to clarify relationships. Clinically, this lateral perspective is the one you want when teaching how eversion strength and lateral ankle stability depend on a smooth retromalleolar course of the fibularis tendons beneath the superior and inferior fibular retinacula. Tendon subluxation after an inversion injury, longitudinal split tears, and painful stenosing tenosynovitis all localize to the posterior aspect of the lateral malleolus, exactly where brevis is constrained against bone. The distal insertion on the tuberosity of the fifth metatarsal also helps explain avulsion patterns at the base of the fifth in forceful inversion and plantarflexion, a common point of confusion with Jones fractures.
