Description
Lateral anatomy of the male lower leg centers on the fibularis (peroneus) brevis in the lateral compartment, its muscle belly lying deep and slightly anterior to fibularis longus along the distal half of the fibula. Distally, the fibularis brevis tendon narrows and tracks posterior to the lateral malleolus in the retromalleolar groove, sharing this fibro-osseous tunnel with the more posterior fibularis longus tendon. Superior and inferior fibular retinacula span from the distal fibula to the calcaneus and fascia, forming the roof over the tendons as they curve inferiorly toward the lateral foot, while the tibia, fibula, talus, and calcaneus define the ankle mortise and hindfoot landmarks. Bone and tendon landmarks are clear. This lateral perspective matters because the fibularis brevis is the peroneal tendon most often involved in longitudinal split tears, typically at the level of the lateral malleolus where the tendon is constrained by the retinaculum and compressed between bone and the fibularis longus. Sports-related inversion sprains can injure the superior fibular retinaculum, allowing peroneal tendon subluxation or dislocation and producing posterolateral ankle pain and snapping that clinicians need to differentiate from isolated anterior talofibular ligament injury.
