Description
Running along the posterolateral compartment of the male leg, the fibularis (peroneus) brevis is presented deep to the fibularis longus, with its muscle belly arising from the distal two thirds of the lateral surface of the fibula. Proximally, the fibular head sits lateral to the tibial condyle at the level of the knee, establishing the bony line the peroneal muscles track as they course inferiorly. Distally, the brevis tendon narrows and passes posterior to the lateral malleolus in close apposition to the longus tendon, both held within the retromalleolar groove. Relative depth is the teaching point here. Clinically, this posterior perspective clarifies why fibularis brevis is the tendon most often involved in longitudinal split tears behind the lateral malleolus, where crowding, a shallow fibular groove, or superior peroneal retinaculum injury can permit subluxation. The relationship to the fibula also matters for operative planning in lateral malleolar fracture fixation and in tendon debridement or repair, where the surgeon must distinguish brevis from longus and protect the common fibular (peroneal) nerve near the fibular neck. For biomechanics, the line of pull explains the brevis bias toward foot eversion and stabilization of the lateral ankle during gait. Educators will find this useful in lower limb anatomy labs, sports medicine teaching on lateral ankle pain, and in orthopaedic or podiatry texts describing peroneal tendon pathology, retromalleolar anatomy, and surgical approaches to the lateral ankle.
