Description

Running along the lateral compartment of the male lower leg, the fibularis (peroneus) brevis muscle belly lies anterior to the fibula’s posterior border and deep to the fibularis longus, tapering distally into a tendon. The tendon tracks inferiorly and posteriorly toward the ankle, passing behind the lateral malleolus in a common synovial sheath with the fibularis longus and held in place by the superior and inferior fibular retinacula. Distal to the malleolus it turns anteriorly to insert on the tuberosity at the base of the fifth metatarsal, lateral to the talus and calcaneus that form the subtalar region. Spatially, this relationship matters because the fibularis brevis is the primary evertor that directly opposes inversion at the rearfoot, so its tendon sits at the front line of lateral ankle sprain mechanics. A short step off a curb can overload it. Forced inversion with plantarflexion can strip the retinaculum and allow peroneal tendon subluxation over the posterior fibular groove, while chronic friction can produce longitudinal split tears that are often difficult to distinguish clinically from isolated anterior talofibular ligament injury. The insertion at the fifth metatarsal also links the tendon to avulsion fractures at the tuberosity, a common point of confusion with true Jones fractures on radiographs.
