Description
Running along the lateral compartment of a male lower leg, the fibularis (peroneus) brevis muscle is isolated beneath the bulk of fibularis longus, tracking inferiorly along the posterolateral surface of the fibula. Its tendon courses distally behind the lateral malleolus in the fibular (retromalleolar) groove, then turns anteriorly toward the lateral border of the foot to insert on the tuberosity at the base of the fifth metatarsal. Proximal and distal tibiofibular relationships frame the muscle’s path, with the fibula positioned lateral to the tibia and forming the lateral buttress of the ankle mortise. Clear. Emphasizing fibularis brevis matters because it is the principal evertor with a direct, palpable distal insertion that frequently becomes a pain generator after inversion ankle sprain. Strain or split tearing of the brevis tendon often occurs where it is compressed between the fibula and the overlying fibularis longus tendon as both pass beneath the superior peroneal retinaculum, and the illustration’s lateral-to-distal tracking helps explain why retinacular injury can permit peroneal tendon subluxation. The fifth metatarsal attachment also anchors clinical teaching around avulsion fractures at the tuberosity (often misclassified as a Jones fracture) and why persistent lateral foot pain can reflect traction at the brevis enthesis rather than midfoot pathology.
