Description
Anterior forearm anatomy centers on the flexor digitorum superficialis (FDS), occupying the intermediate layer of the flexor compartment deep to the pronator teres and flexor carpi radialis, and superficial to flexor digitorum profundus. From a broad humeroulnar and radial origin, its muscle belly tapers into four tendons that run distally in the midline, passing deep to the flexor retinaculum within the carpal tunnel before diverging toward digits II to V. The tendons sit anterior to the metacarpals and proximal phalanges, then split at the level of the proximal phalanx (Camper chiasm) to allow the profundus tendon to pass through to the distal phalanx. Orientation is clear. Medially, the flexor carpi ulnaris frames the ulnar border, while the palmaris longus (when present) tracks superficially toward the palmar aponeurosis. This anterior emphasis matters because FDS is both a landmark and a functional divider in the volar wrist and palm, where its tendons share tight confines with the median nerve and synovial sheaths in carpal tunnel syndrome and tenosynovitis. Isolated FDS testing, holding adjacent fingers in extension while flexing the proximal interphalangeal joint of a single digit, remains a classic bedside maneuver to distinguish superficialis function from flexor digitorum profundus, and it guides assessment after zone II (“no man’s land”) flexor tendon lacerations. Its split tendon anatomy also explains why partial lacerations can still permit some PIP flexion, yet predispose to triggering or adhesions after repair. Ideal applications include gross anatomy lab manuals for upper limb dissection, hand therapy and orthopedic references discussing flexor tendon injury zones, and surgical education materials for open carpal tunnel release and volar approaches to the digits.
