Centered in the lesser pelvis, the urinary bladder (vesica urinaria) sits inferior to the uterus and anterior to the vagina and rectum, with its apex directed anterosuperiorly toward the anterior abdominal wall and its base (fundus) facing posteroinferiorly. Ureters enter the posterolateral bladder wall obliquely, creating the ureteric ridge and defining the superolateral corners of the trigone on the internal surface. Along the inferior aspect, the bladder neck narrows toward the internal urethral orifice, continuous with the female urethra; laterally, pelvic sidewall vessels and autonomic nerves course toward the visceral plexuses adjacent to the bladder. This relationship is the one you need when teaching continence and pelvic organ support, because bladder position shifts with filling and with uterine enlargement, yet the ureterovesical junction remains a fixed surgical hazard.
