Description
Centered in the upper cervical spine, the atlas (C1) sits superior to the axis (C2), with C2 distinguished by its robust pedicles, laminae, and posterior spinous process. From a posterior oblique to posterior view, the vertebral arches frame the large vertebral foramen, while the transverse processes project laterally and the bifid spinous processes become more apparent from C3 to C6. Blue coloration isolates C1 and C2 from the remaining cervical vertebrae (C3 to C7), making their lateral masses and articular facets easier to track in relation to the midline. Rotation of the head occurs primarily at the atlanto-axial joint, so clear depiction of the axis matters when you are teaching cervical kinematics or planning around unstable injuries. Odontoid fractures, traumatic spondylolisthesis of C2 (hangman fracture), and atlanto-axial instability in inflammatory arthropathies all hinge on understanding the spatial relationship between the C2 body, posterior elements, and the C1 ring. The thick C2 pedicles and laminar architecture are key landmarks for posterior C1 to C2 fusion constructs and for safe screw trajectories when the vertebral artery is at risk in the transverse foramen corridor.
