Description
Centered on the deep posterior compartment of the male leg, the tibia lies medial to the fibula with the interosseous membrane spanning between them, while the tendon of tibialis posterior courses distally behind the medial malleolus toward the plantar midfoot. Proximally, the knee region is partially included with the proximal tibia and fibular head as bony landmarks, and distally the ankle mortise frames the talus as the tarsals, metatarsals, and phalanges come into view. From an anterior-facing presentation of both lower legs, the posterior tendon pathway is inferred by its relation to the medial tibial border and the medial ankle structures. Deep anatomy. Clear landmarks. Tibialis posterior is the principal dynamic support of the medial longitudinal arch, and its tendon’s turn around the medial malleolus is the segment most implicated in posterior tibial tendon dysfunction, a common cause of adult-acquired flatfoot deformity. This anatomy also matters in tarsal tunnel syndrome, where the tibial nerve and posterior tibial vessels travel adjacent to the tendon behind the flexor retinaculum, and in medial malleolar and distal tibial fractures where swelling or callus can compromise the tendon’s glide. The bony relationships shown here make it easier to teach why tenosynovitis and attritional tearing cluster at the retromalleolar groove.
