Description
Arising from the superior mental spines (genial tubercles) on the lingual surface of the anterior mandible, the paired genioglossus muscles expand posteriorly and superiorly into a broad fan within the floor of the mouth. Superior fibers course toward the dorsum of the tongue, while inferior fibers descend toward the hyoid region, forming a muscular sling that sits medial to the mandibular body and anterior to the oropharyngeal airway. Adjacent tongue musculature such as the hyoglossus is positioned more posterolateral, with its fibers running from the greater horn and body of the hyoid toward the lateral tongue, helping define the lateral boundary of the lingual space. Teeth and the mandibular alveolar process anchor the scene. Orientation is immediate. Genioglossus matters because it is the dominant protrusor of the tongue and a primary upper-airway dilator, a relationship that becomes clinically concrete in obstructive sleep apnea where reduced genioglossus tone contributes to posterior tongue collapse against the soft palate and posterior pharyngeal wall. Surgeons and proceduralists also track it as a landmark when working in the sublingual and submental regions, since it lies deep to the oral mucosa and sublingual space and can be encountered in approaches for ranula management, floor-of-mouth mass excision, or midline genioglossus advancement procedures. Its mandibular origin at the genial tubercles also explains why anterior mandibular trauma or osteotomy planning can alter tongue posture and airway caliber.
