Description
Arising from the superior mental spines (genial tubercles) on the inner surface of the mandibular symphysis, the paired genioglossus muscles fan posteriorly and superiorly into the bulk of the tongue, forming a prominent extrinsic muscle mass in the floor of the oral cavity. From a lateral, slightly oblique skull and upper cervical spine view, the tongue musculature sits anterior to the oropharyngeal airway space and superior to the hyoid region, with the mandible framing it anteriorly and the cranial base and cervical vertebrae aligned posteriorly. Bony landmarks such as the coronal, sagittal, and lambdoid sutures, the external auditory meatus, and the atlas (C1) and axis (C2) provide fixed reference points for orienting the soft tissues in true anatomical position. Small, light blue elements adjacent to the cervical column suggest nearby neurovascular or ligamentous structures without competing with the primary focus on the genioglossus. Genioglossus matters because it is the principal tongue protrusor and a major upper airway dilator, and loss of its tonic activity during sleep contributes to pharyngeal collapse in obstructive sleep apnea. This angle helps clarify how mandibular position and tongue base posture relate, a relationship exploited clinically in mandibular advancement devices and in surgical planning around the genial region.
