Description
Running longitudinally along the posterolateral lumbar spine, the iliocostalis lumborum is presented as the lateral column of the erector spinae, positioned lateral to longissimus and medial to the abdominal wall musculature. Its inferior fibers arise from the iliac crest and adjacent thoracolumbar fascia, then course superiorly to attach to the angles of the lower ribs. A posterior view also logically includes adjacent superficial layers such as trapezius superiorly and latissimus dorsi inferolaterally, with the deltoid contour visible at the shoulder. Orientation is clear. As the most lateral erector spinae component, iliocostalis lumborum is a common generator of posterolateral low back pain and a frequent target in physical exam discussions of paraspinal tenderness and spasm, where palpation occurs just lateral to the spinous processes and medial to the costal margin. This relationship matters during regional anesthesia as well, because ultrasound-guided erector spinae plane blocks place local anesthetic deep to the erector spinae at the transverse processes, and the spread can track along fascial planes that include iliocostalis. For spine biomechanics teaching, the muscle’s oblique rib attachments make it a clean example of how unilateral contraction drives ipsilateral lateral flexion while bilateral activity contributes to lumbar extension and posture.
