Description

Posteriorly, the paired kidneys lie retroperitoneal on either side of the vertebral column, with the right kidney positioned slightly inferior to the left under the lower ribs. Each kidney’s convex lateral border faces posterolaterally, while the medial border turns toward the midline where the renal hilum would be located, even if not fully exposed in this external view. Inferiorly, the ureters emerge from the region of the renal pelvis and descend vertically toward the pelvis, paralleling the psoas margins. A semi-transparent integument layer keeps the focus on organ position rather than surface back anatomy. That posterior relationship is the one clinicians think in during flank pain, trauma, and operative planning. Percutaneous nephrostomy and percutaneous nephrolithotomy access the collecting system from the back, and success depends on appreciating the kidney’s oblique long axis, the protection of the 11th and 12th ribs, and the route of the ureter as it leaves the ureteropelvic junction. Stone disease, pyelonephritis, and hydronephrosis are discussed at the level of nephrons and renal function, but the patient’s symptoms and the procedural corridor are grounded in gross renal anatomy. Orientation matters.
