Description
Centered on the male larynx, the mucosa forms a continuous epithelial lining over the endolaryngeal surface of the thyroid cartilage anteriorly and the cricoid cartilage inferiorly, draping posteriorly around the arytenoid cartilages and into the interarytenoid region. Superiorly, the mucosal reflection would be expected to continue toward the laryngeal inlet, while inferiorly it transitions toward the subglottic airway. Within this sheath sit the vocal folds and adjacent vestibular folds, with the mucosa spanning between ligamentous attachments to the inner perichondrium and to the conus elasticus. Cartilage, muscle, and membrane relationships are presented in clear layered order. Mucosal integrity and mobility are the main teaching points here, because phonation depends on a pliable cover over the vocal ligament and thyroarytenoid muscle while airway protection relies on intact supraglottic lining. Edema in the loose submucosa of the supraglottis, as in acute epiglottitis or allergic laryngeal swelling, narrows the airway rapidly compared with the tighter subglottic mucosa. For transoral microlaryngoscopy and endoscopic biopsy, this perspective helps orient the surgeon to where mucosa is tightly adherent over cartilage versus where it lifts more readily, a practical distinction when planning a subepithelial cordectomy or assessing early glottic carcinoma spread along the free edge of the true vocal fold.
