Description
Posterior surface anatomy is paired with a transparent organ overlay, placing the right and left lungs within the thoracic cavity deep to the scapulae and posterior ribs. The right lung sits slightly more superior and broader, its oblique and horizontal fissures separating superior, middle, and inferior lobes, while the left lung shows a single oblique fissure dividing superior and inferior lobes and leaving the cardiac notch anteriorly. Hilar regions align near the T5 to T7 levels, where the main bronchi enter medially and branch toward segmental bronchi within each lobe. Abdominal viscera appear inferior to the diaphragmatic domes, offering landmark continuity from lower thorax to upper abdomen. Clear lobar boundaries. This posterior overlay matters when teaching or planning care in bodies with increased subcutaneous adipose tissue, where surface landmarks become less reliable and the posterior lung fields remain a common site for dependent atelectasis and basal pneumonia. It also supports clinical correlation with posterior auscultation and percussion points: lower lobes dominate the back, so breath sound changes below the inferior angle of the scapula often localize to lower-lobe pathology rather than middle-lobe disease. The fissure mapping helps explain why pleural effusions layer posteroinferiorly and why thoracentesis targets the posterolateral costophrenic recess, typically one to two interspaces below the fluid level while staying above the rib to protect the intercostal neurovascular bundle.
