Description
Positioned deep in the posterior upper neck, the rectus capitis posterior major runs from the spinous process of the axis (C2) superolaterally to its broad insertion on the lateral part of the inferior nuchal line of the occipital bone. Lateral landmarks such as the auricle help orient the skull base, while more superficial layers, typically including splenius capitis and semispinalis capitis, sit posterior and superficial to the suboccipital triangle. Medially, the paired rectus capitis posterior majors flank the midline nuchal ligament and occipital crest, forming a muscular corridor over the posterior atlanto-occipital region. Fiber direction is easy to read. This muscle matters because it anchors rotation and extension across the C1 to C2 to occiput complex, and its intimate relationship to the C1 dorsal ramus (suboccipital nerve) makes it a practical landmark during suboccipital dissection.
