Framed from a posterior perspective, the occipital bone and posterior elements of the upper cervical spine anchor a paired set of rectus capitis posterior major muscles in the suboccipital region. Each muscle runs superomedially from the spinous process of the axis (C2) to the lateral part of the inferior nuchal line on the occipital bone, flanking the midline over the posterior arch of the atlas (C1). Inferiorly, the upper thoracic spinous processes and posterior ribs enter the field, with the scapulae positioned laterally and the acromia projecting toward the shoulder girdle. Clean landmarks. Rectus capitis posterior major matters because it defines the superficial medial boundary of the suboccipital triangle and sits adjacent to structures that drive common posterior neck pain patterns. The dorsal ramus of C1 (suboccipital nerve) courses in this compartment, and the vertebral artery loops across the posterior arch of C1 before entering the foramen magnum, a relationship surgeons and interventionalists keep in mind during posterior C1–C2 instrumentation and targeted injections. Its line of pull also makes it a prime teaching example for coupled upper cervical extension and ipsilateral rotation, and for distinguishing suboccipital muscles from deeper cervical extensors on a model.
