Description
Arising from the posterior tubercle of the atlas (C1), the rectus capitis posterior minor courses superiorly and medially to attach on the inferior nuchal line and adjacent squamous part of the occipital bone, just lateral to the external occipital crest. Seen from a posterior perspective, it sits deep at the craniovertebral junction, immediately inferior to the occiput and posterior to the atlanto occipital joints, with the posterior arch of C1 and the spinous process of C2 (axis) forming key bony references. Small and close to midline, it lies medial to the obliquus capitis superior and superior to the deeper posterior atlanto occipital membrane. Tight quarters. This posterior view matters because the suboccipital region is where anatomy feels deceptively simple until you have to navigate it clinically, for example when explaining cervicogenic headache patterns or planning posterior approaches that work around the vertebral artery as it loops over the atlas in the suboccipital triangle. Rectus capitis posterior minor also has recognized myodural connections to the spinal dura, a teaching point often raised when discussing occipital pain with upper cervical dysfunction and when differentiating muscular tenderness from neuralgia at the occipital base. Use it in gross anatomy labs and neck anatomy lectures to orient learners to the deep suboccipital layer before layering in rectus capitis posterior major, the obliquus capitis muscles, and the greater occipital nerve emerging inferior to obliquus capitis inferior, and in surgical education materials covering posterior C1 to C2 exposure and safe landmarks near the inferior nuchal line.
