Description
Arising from the posterior tubercle of the atlas (C1), the rectus capitis posterior minor courses superiorly and slightly medially to insert on the medial part of the inferior nuchal line and adjacent squamous occipital bone. A lateral perspective places the muscle deep to the trapezius and splenius capitis region, immediately posterior to the atlanto-occipital joint and just inferior to the occiput, with the mastoid portion of the temporal bone and the upper cervical spinous processes serving as bony orientation points. Nearby, the posterior arch of C1 and the laminae of C2 frame the suboccipital compartment. Small muscle. Big landmark. That short fascicle matters because it is one of the few muscles spanning the atlanto-occipital interval without attaching to the axis, so it is taught as a fine control structure for head extension and as a reference when mapping the suboccipital triangle and the greater occipital nerve (C2) as it emerges inferior to obliquus capitis inferior. In headache and neck pain clinics, the rectus capitis posterior minor is often discussed alongside the myodural bridge, a fibrous connection from suboccipital musculature to the spinal dura that can be tensioned by sustained upper cervical extension or postural load. Surgeons and interventionalists also use these layers to anticipate the depth and trajectory for occipital nerve blocks and posterior C1 to C2 approaches.
