Description
Positioned at the skull base, the rectus capitis posterior minor runs from the posterior tubercle of the atlas (C1) superiorly to the medial part of the inferior nuchal line on the occipital bone. Medial to the rectus capitis posterior major and deep to semispinalis capitis, it occupies the most medial corner of the suboccipital region, where the dorsal arch of C1 and the posterior atlanto-occipital membrane form key adjacent landmarks. A lateral neck context is also suggested by inclusion of the external ear and cranial base, and by a contrasting, pink-highlighted stylopharyngeus descending from the styloid process toward the pharyngeal wall, anterior and medial to the cervical paraspinal musculature. Small bones matter here. For teaching and surgical planning, this is the view that helps you orient the suboccipital muscles against the occiput and the upper cervical vertebrae, a necessary step before discussing posterior approaches to the craniovertebral junction and the mechanics of atlanto-occipital extension. Rectus capitis posterior minor also has clinical relevance through its fascial connections to the posterior atlanto-occipital membrane and dura (often described as part of the myodural bridge), a relationship commonly raised when correlating cervicogenic headache patterns with upper cervical muscle tension.
