Description
Radiating from the pulmonary trunk, the right and left pulmonary arteries course laterally from the superior aspect of the heart toward their respective hila, then branch into lobar and segmental arteries directed to the upper, middle (right), and lower lobes. An x-ray style, semi-transparent treatment of the cardiac silhouette typically preserves the mediastinal landmarks, with the right pulmonary artery passing posterior to the ascending aorta and superior vena cava, while the left pulmonary artery arches over the left main bronchus. Distal arborization into smaller arteries and arterioles is suggested by tapering, tree-like vessels as they run inferiorly and laterally into the lung fields. Clear spatial logic matters here. For clinical teaching, this projection-oriented anatomy is central to interpreting pulmonary embolism on CT pulmonary angiography, where filling defects often lodge at lobar or segmental branch points and can preferentially involve the lower lobes due to flow distribution. It also supports procedural planning for pulmonary endarterectomy and for catheter-directed thrombectomy, where recognizing the relationship of the main pulmonary arteries to the aortic root, left atrium, and bronchi reduces orientation errors. Congenital variants, including unilateral pulmonary artery hypoplasia or anomalous origin of a pulmonary artery from the ascending aorta, are easier to explain when the normal branching pattern is laid out in a radiograph-like view.
