Description
Circumferential layers of the small intestinal wall are rendered in cross-section, from mucosa with plicae circulares and villous mucosal contour toward the submucosa, then outward to the muscularis externa with inner circular and outer longitudinal smooth muscle. A thin subserosa of loose connective tissue sits superficial to the muscularis, deep to the serosa, and it is in this plane that larger mesenteric vessels and accompanying nerves begin to penetrate toward the bowel wall. Arterial and venous channels course in the subserosal connective tissue before sending branches medially into the muscularis and submucosa, with lymphatic channels tracking alongside. Attention to the subserosa matters because it is the transition zone between mesentery and bowel wall, the point where vasa recta and neurovascular bundles commit to an intramural path. During resection of jejunum or ileum for ischemia, Crohn disease, or strangulated obstruction, surgeons judge viability and plan mesenteric division by following these subserosal vessels and their branching pattern, while aiming to preserve marginal perfusion at the cut edge. It is also the layer where edema, hemorrhage, or inflammatory spread can dissect, and where serosal inflammation can tether adjacent loops.
