Description
Deep to the mucosa of the small intestine, the submucosa (tela submucosa) forms a dense connective tissue sleeve between the muscularis mucosae and the muscularis externa. A circumferential lumen sits centrally, lined by mucosal folds with villi projecting into the cavity, while the thicker muscularis externa lies peripheral to the blue-stained submucosal layer in distinct inner circular and outer longitudinal strata. Small arteries and veins course within the submucosa and send branches toward the mucosa, and nerve bundles track in parallel before distributing to the plexuses of the gut wall. Clean lamination is the point. Submucosal architecture matters because it is where the bowel wall gains tensile strength while still allowing the mucosa to slide over the muscularis during peristalsis, and it is also the conduit for larger-caliber vessels and lymphatics that feed the villi. In the duodenum, Brunner glands occupy this layer and explain why duodenal submucosa looks and behaves differently from jejunum and ileum, including the classic teaching distinction between mucosal villi and submucosal glands in proximal small bowel histology. Clinically, submucosal edema and hemorrhage thicken the wall in ischemia and inflammatory enteritis, and submucosal tumors such as gastrointestinal stromal tumors or neuroendocrine lesions often present as smooth mucosal bulges on endoscopy because the mucosa remains secondarily draped over the lesion.
