Running along the prevertebral fascia, the cervical sympathetic trunk ascends posterior to the carotid sheath and expands superiorly into the superior cervical sympathetic ganglion at the level of C2 to C3, deep to the upper sternocleidomastoid. Nearby landmarks in the lateral head and neck include the angle of the mandible and mastoid region, the parotid-masseteric area, and the exposed skull and mandible supporting the facial musculature. Yellow peripheral nerves course across the face and neck in the superficial planes, while the ganglion is singled out as a small blue focus in the upper neck. Relationships matter here: the ganglion sits medial to the internal jugular vein, posterior to the internal carotid artery, and anterior to the longus capitis and longus colli muscles. For teaching sympathetic pathways, this is the node you want. Postganglionic fibers leaving the superior cervical ganglion form the internal carotid plexus to reach the orbit and cavernous sinus (classically implicated in Horner syndrome with ipsilateral ptosis and miosis) and the external carotid plexus to supply facial sweat glands and vasomotor tone, while gray rami communicantes distribute sympathetics to the upper cervical spinal nerves.
