Description
Arising from the common tendinous ring at the orbital apex, the superior rectus muscle courses anteriorly along the roof of the orbit to reach the sclera on the superior aspect of the globe, lying deep to the frontal bone and superior orbital plate and superficial to the optic nerve. In a lateral skull view the red fascicles can be appreciated within the orbit as they run parallel to the orbital roof, with the tendon narrowing as it approaches its insertion anterior to the equator of the eyeball. Medially, the muscle tracks toward the region of the trochlea of the superior oblique, while laterally it relates to the lacrimal gland fossa and the lateral rectus compartment. Functionally, superior rectus is the primary elevator of the eye, but its line of pull also produces adduction and intorsion, a relationship that becomes clinically obvious when you test elevation in abduction to isolate the muscle from inferior oblique contribution. This is a key anatomic point in diplopia workups and in differentiating oculomotor (CN III) palsy from restrictive disease such as thyroid eye disease, where the inferior rectus often limits upgaze. Surgeons also care about its proximity to the levator palpebrae superioris and the superior ophthalmic vein at the orbital apex. Small space, big consequences.
