Description
Cranial sectioning exposes both orbits with the superior rectus muscle coursing in a straight line along the superior aspect of the globe, running anteriorly from the common tendinous ring at the orbital apex toward its scleral insertion just posterior to the corneoscleral junction. The optic nerve (CN II) exits the posterior pole of each eyeball and tracks posteriorly through the optic canal, while the superior rectus sits superior to the nerve and medial to the lacrimal gland region. Portions of the other extrinsic extraocular muscles are suggested around the globe, but the superior rectus dominates the superior orbital compartment. Teeth and maxillofacial structures remain intact inferiorly, anchoring the orientation of the skull in a neutral frontal position. Superior rectus anatomy matters when teaching vertical gaze mechanics and when localizing oculomotor (CN III) dysfunction, since this muscle is innervated by the superior division of CN III and weakness produces impaired elevation, often most evident in abduction. Clinically, the relationship to the optic nerve and orbital apex helps frame discussions of orbital apex syndrome, compressive lesions, and surgical corridors that must respect the tendinous ring and adjacent neurovascular structures.
