Description
Running along the medial wall of the orbit, the medial rectus muscle is presented from an anterior viewpoint within a skull section, its fleshy belly hugging the lamina papyracea of the ethmoid and the adjacent frontal process of the maxilla. From its posterior origin at the common tendinous ring (anulus tendineus communis) near the orbital apex, the muscle courses anteriorly and inserts on the sclera just medial to the corneal limbus. The globe sits centrally, while the remaining extraocular muscles are partially visible around it, framing the anterior segment and defining the rectus cone. Medial lies the nasal orbital wall, lateral lies the globe and intraconal space. Medial rectus anatomy matters because it is the prime adductor of the eye and one of the extraocular muscles most often implicated in horizontal strabismus. Oculomotor nerve (CN III) innervation and the muscle’s tight relationship to the medial orbital wall explain common patterns of limitation after medial wall blowout fractures or post-ethmoidectomy scarring, and they also guide surgical planning for medial rectus recession or resection. Short statement: this is a key landmark. The anterior vantage is also useful for teaching how the rectus insertions form the Spiral of Tillaux and why misplacement during strabismus surgery can create incomitant diplopia.
