Description
Centered in a lateral view of the cervical spine, the third cervical vertebra (C3) sits inferior to the atlas and axis and superior to C4, with its vertebral body anterior and the spinous process projecting posteriorly in the midline. The vertebral foramen lies posterior to the body, forming the osseous canal for the cervical spinal cord. Lateral to the body, the transverse processes carry the foramen transversarium, and the superior and inferior articular processes form the zygapophysial joints that link adjacent vertebrae. Short pedicles and thin laminae bridge the body to the posterior elements. C3 matters because small changes in alignment here can have outsized neurologic consequences. Degenerative spondylosis at C3 to C4, including uncovertebral joint osteophytes and facet arthropathy, can narrow the spinal canal or intervertebral foramen and contribute to cervical myelopathy or C4 radiculopathy patterns, while high-energy flexion extension trauma can produce instability that is assessed on lateral radiographs and CT. Clear appreciation of the vertebral foramen and posterior elements also underpins safe placement of posterior instrumentation, where the course of the vertebral artery through the transverse foramina limits lateral breach. Faculty often select this type of gross anatomy plate for head and neck blocks, vertebral column labs, and radiology correlation sessions when introducing cervical levels and counting landmarks on a lateral view.
