Description
Centered within the anterior thorax, the heart is overlaid on the left side of the sternum, with its apex pointing inferolaterally toward the left fifth intercostal space. The right atrium and right ventricle occupy the more anterior cardiac surface, while the left ventricle forms the left border and the apical contour; the left atrium sits posteriorly and is only suggested by its superior outline. Superiorly, the great vessel origins are implied at the base of the heart, deep to the manubrium. Subcutaneous adipose tissue of the chest wall and upper abdomen frames the cardiac silhouette. Obesity changes the teaching context of cardiac surface anatomy because a thicker anterior chest wall and increased epicardial fat can make landmarks less intuitive and can blunt palpation of the apical impulse on physical exam.
