Description
Posterior thoracic anatomy is rendered as a semi-transparent overlay on the back of an obese adult white male, placing the heart centrally in the middle mediastinum and slightly left of the midline as it lies against the diaphragm. The atria sit superior and posterior to the ventricles, with the left ventricle forming much of the left-sided cardiac silhouette, while the right ventricle remains more anterior and less dominant from a dorsal viewpoint. Coronary anatomy is emphasized at the atrioventricular groove (coronary sulcus), which encircles the heart and separates atrial tissue superiorly from ventricular myocardium inferiorly, a consistent external landmark even when surface adiposity is present. Faint, paired lungs flank the heart laterally, and portions of the upper gastrointestinal tract appear inferior to the diaphragm in posterior projection. A posterior, through-body perspective clarifies where the heart actually sits relative to the lungs and vertebral column, a point that students often miss when they only see isolated cardiac specimens. Body habitus matters clinically. Obesity can increase epicardial adipose tissue and can make echocardiographic windows more limited, so understanding the true posterior relationships helps when correlating imaging with physical anatomy.
